Position the E-Z Graph® with the dot centered in the wound. Trace the wound margins using a BLUE marker, erythema using a RED marker, and eschar using a BLACK marker. Peel away and discard the soiled plastic backing. Place the self-adhering traced portion of the graph onto the front side of the worksheet. Complete the top part of the E-Z Graph® by filling in or circling appropriate information using the black marker. Complete the back page of the worksheet using an ink pen.
